2012 Care Information

We are committed to providing a safe environment for young people throughout all of our programs.

In order to assist us, we ask that parents/carers of students who regularly attend Gateway Youth complete a Care and Information Card to help us care for students' specific needs.

If your child attends Gateway Youth, please download, complete and return the form below. You can return it on Friday Nights, Sundays at Church, or through the mail (Youth Department, Gateway Baptist Church, PO Box 2033, Mansfield Q 4122).
